All Advanced Airway procedures: i.e. OPA, NPA, ET, LMA, Combitube, Cricothyrotomy, including MEMS QA Form. This form requires Acrobat Reader

All CPAP utilization

Remember, even though your are using electronic run reporting, you still need to send me your 12-lead ekg's with a completed QA form. The same applies for airway responses.

QA Officers: if you do not have any runs that meet the indicators, please notify me of that, thank you.

Remember if your service is more than three (3) months behind in QA, the Regional Medical Director will not sign your license application.
